Yes, widely. Guatemala is predominantly cash-based. Local markets, chicken buses, comedores (local eateries), Lake Atitlán villages, and most businesses outside Antigua and Guatemala City require quetzal cash.
ATMs and withdrawing cash
ATMs are available in Guatemala City, Antigua, and larger tourist towns. Banco Industrial and BAM machines accept foreign cards.

- Lake Atitlán villages have very limited ATM access
